  2. Willingness to relocate. If so, where would you like to practice? Which state?
  3. Are you Board Certified or Board Eligible? 
  4. Do you have any limitations placed on your license to practice?
  5. Your last employer. Duration of employment.
  6. Your salary and benefits expectations.
  7. 3 References.
  8. Part time or full time employment preference.
  9. Travel restrictions or preferences.
